Paranoia form pack

A long timecycle ago in the depths of Alpha Complex I seem to fondly remember an expansion to Paranoia entitled Paranoia Form Pack. I remember fondly filling out the different forms completely and in triplicate. I especially remember, with great fondness, filling out the form request form one more than one occasion. Any chance of that suppliment ever re-seeing the light of the upper levels of Alpha Complex?

mmm... yes all that paranoid buracratic goodness...

At a convention last summer I managed to pick up the entire 2nd edition of Paranoia, including a fairly complete set of forms (multiple copies, never used etc.) and the 6-issue limited Paranoia comic book.
