Would it be possible to make up a summative feedback sheet as a multi-choice questionnaire plus additional comments boxes?

They did it with the D&D playtest, and it just strikes me that while all commentary is valid, a few strong opinions don't always represent a majority view. Moreover, it helps clarify what areas the game design team really wants us to focus on.

The multi-choice could be done as a poll on the forum.

There may be specific areas of the rules where we do this, but I don't want to do it as a general/global poll.
