however I've just one criticism about the sheets: the severed heads on the combat record page make the last two line almost unusable.
I don't know what the status with the French editor Le Grimoire is but they create a very nice sheet for their homebrew "heroic system" also based on the LW game book system. It's in French but I like the design. it can be found here.

... Because I honestly think its more useful than these but still uses the 'official' action chart style. The ones you've posted don't look any different in utility to the ones in the book - ie, they are a little prettier but still have no real place to put special items information or notes on disciplines, there's wasted space regarding combat record/ratio history, and no information relating to player name or any other personal details. Also, the design setup lends itself to a landscape design and these are setup potrait so you get that break-up on the scroll work as it goes across two pages.

That's not a dig at Mongoose - I did spend a few hours on them and put some thought into their content and organisation.
