does the télépathy check is made with the full telepathy bonus (charisma modifier + feat bonus + telepathy skill ranks, etc.) or just a flat D20 ?

Since skilled telepath control much better their ability, it would seem logical than thay control better the risk of avoidinf Accidental Scan.

It's a straight Telepathy check, with all bonuses.

If you want a more complex rule that reflects the series a bit better, then you could change it to a check of 1d20+Charisma+P-rating vs a DC of 10+the character's Telepathy skill ranks. It's a bit more fiddly, though.
