Yeah but where do you draw the line?

From your post, it seems you allow more than a strict read, so you should really answer that question yourself. To me, it's what's reasonable under the circumstances, i.e., part of GMing - the sheathed weapon, the weapon adjacent to the person's hand, etc.
 
Well yeah I do allow looseness in the rules, fun is sometimes more important than order. And flavor is sometimes more important than flavor. It all depends though. I wouldn't allow Quick Draw to prevent an AoO, just because if I did it for the players I'd do it for the bad guys. Most likely is a player wanted to be able to quick draw an opponents weapon or one off the table I'd asked them to write it up as a combat maneuver. Maybe a combo of Improved Initiatve/ Quick Draw or Lightning Reflexes/Quick Draw, and then allow it to quick draw a weapon that's in the person's square would seem fair to me.

Quite understandable, as the default rule is that picking up an item draws an AOO, but I consider that more akin to the person leaning over to pick up an item from the floor or reaching across the table to grab an item, rather than simply grabbing the weapon that is directly adjacent to your hand, e.g., on the table, or if you drop to the ground, on the floor where it landed when you were disarmed.

To me, it comes down to reality, which trumps artificial rules. For instance, the gunslinger could be sitting down playing cards in a Western game, and either have his gun in his holster, in his lap, on the table right next to his hand where he is holding cards. In each case, I would allow for Quick Draw to work, else you end up with ludicrous situations or situations where PCs act in a certain way because of the rules over reality. Namely, according to strict rules, the PC with a gun on the table next to their hand holding cards couldn't quick draw the gun, but could withdraw his hand extended over the table, drop it to his side, and pull out his gun with quick draw. If you draw that line, PCs won't do something cool like set their gun on the table when settling in at the gaming table, but instead will stick with what the rules allow and keep their gun holstered. That doesn't promote fun gameplay to me, nor necessarily equate with reality, so I don't let the rules override either fun or reality.
