Should Advancement be split up?

in the core rules...

advancement raises your rank and also gives you an extra skill

but for some careers like Scholar>Physician where survival is 4+ and advancement is 8+

it makes doctors have less benefits and less skills (except those with a high Edu)

now I understand the tradeoff of survival vs advancement

but I don't quite see the link between getting a second skill and gaining a rank.

Two equally valid explanations have cropped up from my players:
  • Promoted to new job duties, which resulted in learning the new skill (either formally or informally)
  • You'd developed the skill, someone noticed, and you got promoted over it.
