What proof do you have that you have permission to copy this map? And the book? People don't abandon their copyrights, and the owner might be interested to know. Paint me skeptical. If you have some proper permission, then you need to share it. Doing the one map for yourself is actually permitted under copyrihgt rules, but making copies for others is not. Personally, I don't think it is that great a map, although it is well done cartographically speaking.

Then I suggest asking for their authorization to print the map as it is as out of print as the book.
 
The Gazetter is a nice text, I have a couple of copies, and they can be found from time to time on ebay, etc. I'm not sure if I have the map though, so I would be interested in it as well, provided the rights can be demonstrated. I am an intellectual property attorney, so I'm not looking to get sued over this :P If there are some issues to wrap up regarding obtaining rights, feel free to PM me (no guarantees on results though!).
