On first flick through of the pdf, I spotted a problem: the large chapter title and the chapter page header appear on the same page; this problem is present on pdf pages:

Also: on pdf page 91, header text is *behind* the accent bar; it should in front of it.
Quick question on a minor (but, to me, *highly* irritating) point: In the chapter entitled "New Equipment", in both the deadtree book and the pdf, the chapter title says "New Equipment" but the chapter page header says "Equipment" - should this be not be "New Equipment"? Just a thought.........
