Medic!

True, but someone without any medical knowledge should really not try
to treat any injury big enough to have caused a characteristics loss.

Sometimes, there is no choice. Artery cut? No medic? You can call the S.O.L. hotline if you don't want an untrained (MGT definition) to try something. How could they make that one worse?

Nope, that isn't worse than death.
No, but the failed first aid caused additional damage which would have
been avoided with a professional treatment of the injury. And as I see
it the alternatives we discuss are a successful use of first aid versus the
consequences of a failed use of first aid, not first aid versus no first aid.

Bleeding stopped, but too much pressure for too long, resulting in necrosis
and the necessity to amputate the arm or leg.

Nope, that isn't worse than death.

I think the point isn't making it worse than death but that you cause more injury. And someone with a minor wound seeking treatment for it isn't facing immediate death anyway. In such a case botched first aid may result in more damage.

There's really two ways to go with this.

1 - fast and loose, fun trumps reality, everyone lives - ignore failure entirely or at least don't penalize for it

2 - a touch more reality where sometimes the good guys do get hurt and die, despite or even because of aid
