Hit points at first level

I have a question that pertains to Conan, but also to other d20 games.

The "you get maximum hit points at 1st level"-thing; is it meant to be a special bonus for PCs, or to also apply to all NPCs?
I've seen both variants used in sourcebooks (Conan and other d20 stuff), and I'm wondering if there is a canon answer to how this is meant to be handled.

There is certainly a Mongoose 'House style' for creating NPC and Monster hit points. I have reproduced it below for you.

NPC's with PC classes = Maximum hp at 1st level, then rolled randomly for the rest of hit dice.

NPC's with NPC classes (expert warrior etc) = Average hp at 1st level, then rolled randomly for rest of hit dice.

Newly statted monsters (such as found in the monster manual) = All hit dice considered average roll, rounded down eg. 3d10 HD monster would end up as 5.5 + 5.5 + 5.5 = 16.5 rounded down to 16.

Newly statted monsters with PC classes = Maximum hit points at 1st level, then average for all subsequent hd eg. 3d10 HD monster would have hit points of 10 + 5.5 + 5.5 = 21 hp.
