Greetings fellow rulemakers and lawyers

Do the lost actions due to multiple wounds etc stack? E.g. Unlucky Leif is hit in the abdomen and goes to -2 hp there, Leif loses 4 actions, during next action Leif get hit again and his left arm goes down to -1. Does poor Leif, who still has 3 lost actions get 1d4 more lost actions?
And can he use reactions while shocked by his injuries?

A question about item quality. Does the bulwark armor effect add to armour penalties? If Leif had a exquisite helmet (normal ap 5, -5% skills) with bulwark 2 (AP 7), is the helms armor penalty -5% or -7%?

Yes, they stack...once you start losing combat actions, it can get brutal. However, you can still use Reactions during that period.

There's nothing in the RAW to say that the Bulwark bonus is exempt from the Skills penalty, so I guess it would be -7%. However, as a GM I would not increase the skill penalty, as there seems little point in using Bulwark armour otherwise.
