What do you mean by "bluff"? I checked Combat Actions and Manoeuvres but there's no option like that.

first: Yes, you use the combat action to "charge" - closing in and hacking at the opponent, plus some sideeffects. Alternatively you could approach more carefully, using a combat action to "move" close to the opponent and then on your next turn "attack". Unless he's armed with a freaking deadly long spear or something, most of the time just charge.

second: Readying a weapon means "to make it ready for use". Drawing a sword, taking the bow down from your back and so on.
