Ok, so I'm guessing no one else has any idea where to find such a form either. Given that, I threw together a quick and dirty ship form myself. It's nothing special -- and fairly ugly -- but I think it covers all the bases in terms of necessary information. In the spirit of good fellowship, I posted it for public consumption here:
