Aft hatch on plans

Banded Mongoose
I seem to recall that most ships have some sort of aft hatch, allowing the engineering section access to the outside. None of the deck plans seem to have one, though.

Sometimes it's in the deck or overhead, and sometimes it's in the aft bulkhead, but I wouldn't want to be in engineering and not have some emergency way out!

Are you sure you aren't thinking of a different version of Traveller, or perhaps deckplans found online? I checked, because I didn't remember this being that common in MGT, but I have seen it a lot more often in other versions. The only ones I see in the old core rules that have exterior hatches in engineering are the scout/seeker and the free trader. The up/down hatches on the others relate to hardpoints or access to different decks. That said, I'm all for adding more of them, as it makes a lot more sense from a maintenance perspective. Also what if the captain needs to vent engineering because MCGUFFIN?

The original scout/courier from CT/MT had a hatch in the rear of the ship, directly into the engineering spaces.

This link shows a 3D model built to the original scout ship specifications.
