If your Defensive Value is 10 + Dex/Str bonus + dodge/parry bonus what happens when you are caught 'flatfooted'? Do you lose all bonuses?
is everyone considered a 10 DV when surprised?

I mean it does make sense. If a tower guard is completely unaware that an attacker is behind him it should be very easy to hit him. He may very well be a juggernaut in combat when he has all his bonuses but since he is not aware of his attacker he is just as easy to kill as an untrained person.

Yep.

Although, certain classes (Barbarian, Pirate) recieve at 4th level, the special feat of Uncanny Dodge, which allows you to retain your Parry/Dodge bonus.

And later, Improved Uncanny Dodge which means you can never be flat-footed.
