RuneQuest... what setting?

Well, they couldn't call it 'Call of Cthulhu' as that would infringe Chaosium's Trademark, but given the antiquity of the Lovecraft material and lack of any evidence of copyright extension on any of his work, there's nothing to stop them from publishing a horror RPG that borrows heavily from the Mythos.

Arguably there is already pretty much an RQ version of Call of Cthulhu, it's called err... ...Call of Cthulhu. :?

There is a D20 CoC also (not sure if that is included.

That's pretty much dead in the water, it was perhaps predictably enough, badly received. It made Lovecrafts Ghouls "undead" for Azathoth's sake!!!
Well, there was a bit of grumbling from CoC grognards about d20, but it was actually pretty nicely done. I don't know any hard figures but I didn't get the impression that it was a disaster commercially, either - it's not as if there are unsold piles of the thing sitting around mouldering in some warehouse. However, Chaosium utterly dropped the ball on supporting the d20 side of things - the "Pulp Cthulhu" setting book which was meant to accompany it has still not appeared, and is now BRP only, if it ever sees the light of day. Chaosium are a shoestring operation these days and their turnaround times are glacial.

Have you come across HarnWorld? A fairly gritty early medieval style world, which has rules-free supplements and works very well with RQ rules. Monsters are few and far between, magic is low-key, and the level of detail in the supplements is amazing. If you're into fairly realistic role-playing, you might well enjoy it. I believe the original creator had some sort of falling-out with his original publisher, so the older stuff is produced by Columbia Games and the newer stuff by Kelestia Productions.
