Are there any off-hand penalties? Given that a limb could be disabled by a wound, it seems likely that a character may want to continue fighting with the other hand.
I can't recall any rules by RAW. It depends whether the limb has been wounded - which usually puts it out of action - or whether the off-hand is used for something else. I can see any number of rulings ranging from imposing significant penalties to imposing none. Personally, I'd avoid penalising PCs unless it added dramatic tension.
I'd simply impose a "difficult" penalty to combat style ie -20% or "hard" (-40%) in the event of a really specialized weapon. No need for specific rules imo.
