Is the +1 DM for rank optional or must it be applied everytime.

I prefer to leave as many decisions as possible to the players, so I would
let them decide whether and how to use the +1.

As I said in your other poll, I make it optional, after roll. Letting the player decide what benefit they want of the two choices.

If you go strictly by the rules though, I think you have to apply the DM and it is done before the roll.

It's as mandatory as any other part of the rules is.

If the question is asked as a Referee - my answer is optional, after the roll (i.e., how I rule it).

If the question is asked as a player - it is your Referee's decision.
