Is a '1' always a fail?

I am sure I've read this somewhere, but can't think where. But am I right in thinking that there are circumstances in a call to arms where rolling a '1' on a D6 doesn't mean an automatic failed test? Crew Quality tests for example...

No, it only applies for attack rolls. And similarly 6 for automatic hit.

No, precise applies to the bulkhead/standard/critical roll, not the roll to hit.

But a 1 is not always a bulkhead hit on the damage table, so precise weapons can never get bulkhead hits.
