Looks like the first thing we'd call a full-sized UAV (engine, and radio controlled by an operator) was in 1935. I'm not counting things in which the course was pre-determined before launch.
 
All you need is a satellite with sensitive enough equipment.

The difference is that unless you expend limited fuel resources, satellites have a known orbit, whereas reconnaissance vehicles can be discretely inserted.
 
Not all satellites have a fixed orbit and can be moved (Slowly) into another orbit.

Further, there appears to be a sort of hybrid approach with certain satellites where they exhibit fuller range of motion than would be required for orbiting. ( Alleged Russian or Chinese object seen moving between satellites.)
 
Still takes fuel to move a satellite.

Hunter killer satellites tend to be one shot missions, expensive reconnaissance ones aren't moved unless the information is considered critical.

However, the Air Force has been sending up it's spaceplane drone, possibly doing maintenance and refuelling.
