No. You gain a fourth discipline ONLY if you read the previous book. The game rules are written as if every book is your first book. And then you have only three discipline.

Fairly sure 3 is the way it is intended. But notice that ranged attacks (and similar) does not engage someone. So someone who has not been engaged in melee is free to act and attack someone. This because you do not deal damage back to a ranged attacker.

The books are generally error free. The type of errors you are seeing are usually present in earlier books as well, although often they are different errors in those books. There is no such thing as an error free book. They are just as non-existent as error free software programs.
