Severed or Maimed damage

Is a limb severed or maimed if it has been reduced to negative hit points greater than location total on one strike? Or if total combined damage from multiple hits reduces hit point total to negative beyond hit points in location?

I am going to make a house rule where it must be reduced to beyond negative over location in one strike.

However, if anyone can provide a compelling arguement against this house rule, I am open-minded and willing to listen.

How to other DMs handle this query? Also, is there already an official ruling on this somewhere? Please direct me if so.

I'm actually a GM not a DM, but the way I deal with it is:

A limb is always maimed after taking double location hit point damage, even if the damage is from multiple hits.
A limb is severed if that damage was done in one attack with a cutting or slashing weapon.
Also, a limb will be severed if it takes triple location hit point damage in one attack with a crushing or impaling weapon.

EDIT: And of course, a sever to head or torso equals death...
 
If I get a game going that will be my take on it as well. Cumulative damage just renders the limb maimed; greater then double hp damage is a sever,or destroyed beyond function with a bludgeoning weapon.
