Electronic Warfare and Missile Salvos

A.) Somewhere, either CRB2e or HG2e, I read that EW could be performed once per round on a missile salvo, removing a number of missiles equal to the effect of the EW action. 1.) Does anyone know what page/book this was? My search-Fu is lacking at the moment, and 2.) Is it once per ship per round or once per fleet per round?

B.) If a Merc Cruiser launches 12 missiles at a target on the same turn as 2 Scouts launch a missile, all at the same target, is it 1.) 1 salvo of 14 missiles or 2.) 3 salvos, 1 of 12 missiles and 2 of 1 missile?

C.) A salvo of missiles successfully impacts a target, is the damage per missile rolled independently or combined? I would think independently on the reasoning that the missiles are not all going to impact the same part of the ship at the same instant of a 6 minute turn else they'd be bumping into each other. Clarification would be nice here.

B) 2. Each ship fires a separate salvo.

C) I think it's clear; roll for damage once and multiply by number of missiles that hit.
