Duds in space

Boom - Boom... Um, the lights didn't go out?

The article link below discusses a common issue with military ordnance - it doesn't always work as expected. Quality control and other measures (such as detonators, fuzes, etc) have improved since the first lobbed explosive was invented. However I think it might never go away given that Murphy still apparently exists in the Far Future. In a fleet action it's a bit different, but I would expect GM's to use this as a possible plot point. And, technically, one wouldn't know that a warhead wouldn't detonate until it actually hits, so it wouldn't be part of any defending roll checking for misses.

So does anyone ever have it where a missile has made it's way through everything a defender can throw at it and it didn't detonate?

There is a scene in Shadow of the Storm where Stormshadow only has training missiles aboard, so fires them in penny packets to ensure the enemy's point-defence can cope. We don't want one to get through and show them theyre just duds; they might stop being distracted....
