Alternatively, 1st level could be taken as the base point for special forces, and stuff expands from there. This would make more sense to me than forcing the players to start out as raw recruits. After all, 4e seems to have moved the PC power level up from where it was in earlier editions.

I'm honestly not a big fan of the cover art at all. The idea of fantasy special ops is a kickass one, but I'd like a bit more fantasy than M-16s getting replaced with crossbows. One conversion for Paranoia which I've yearned to do was a Warhammer 40,000 riff. Dark Heresy, but playing up the self-defeating, ridiculous, and comitragic nature of the Imperium. Bureaucracy, stifled innovation, and utter paranoia over the nature of the universe...it fits pretty darn well.
